Patients
Preparing for your first appointment
To make the most of your time with us, the following steps should be taken:
- Prior to your
visit, contact your health insurance company to find out if you need
a referral from your primary care physician (your regular doctor). If
you do need a referral, it is important that you get the referral before
your office visit, in order to ensure that your insurance company will
pay for your visit.
- Please arrive
15 minutes early for your first appointment, so that you will have sufficient
time to check-in, complete your new patient paperwork.
- Being on time
for your appointment is greatly appreciated so that you are able to
maximize the time you spend with Dr.Rencic, and so that our other patients
are not inconvenienced.
- In fairness to
our other patients, if you arrive late -- we regret that we may not
be able to accommodate you on that day.
In advance of your visit, you can print out the patient registration
forms, and complete them prior to your visit.
- If your insurance
plan requires a referral from your primary care physician, then you
must obtain this referral before your appointment.
- Please bring your
insurance card to all appointments.
