Preparing for your appointment
To make the most of your time with us, the following steps should be taken:
Prior to your visit, contact your health insurance company to find out if you need a referral from your primary care physician (your regular doctor). If you do need a referral, it is important that you get the referral before your office visit, in order to ensure that your insurance company will pay for your visit. If you are unable to obtain a referral by the time of your visit, you unfortunately may be asked to reschedule your appointment.
Please arrive 15 minutes early for your first appointment, so that you will have sufficient time to check-in, complete your new patient paperwork. Being on time for your appointment is greatly appreciated so that you are able to maximize the time you spend with Dr. Rencic, and so that our other patients are not inconvenienced.
In fairness to our other patients, if you arrive late --more than 15 minutes late to your scheduled appointment-- we regret that we may not be able to accommodate you on that day.
Prior to your visit, you can print out the patient registration forms for completion.
In order to protect the rights of our patients identity we are required by the government to adhere to an Identity Theft Prevention Program (The Red Flag Act). This requires every patient to show a photo ID at the time of the visit.
Please note, Dr. Rencic is not accepting new patients.
